Home Health RN Case Manager - $10K Sign-On Bonus

CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one in-home care, developing personalized plans with physicians and clinicians, and coordinating with a team to support recovery and independence. You will administer medications, perform wound care, and educate patients and families.

The role requires nursing licensure, CPR, and a valid driver’s license. A $10K sign-on bonus is offered and the position is full-time with 40 hours per week.
