Key Attributes and Focus

  • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients.
  • Coaching for health: The PCP acts as a health coach, helping patients set short‑ and long‑term health goals, partnering with them to work toward those goals, and frequently following up to improve health.
  • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is actionable for patients and for care teams in meeting performance goals.

The organization measures metrics such as patient admissions per thousand, appointment utilization (18–21 slots per day), CGCAHPS, clinical gaps closures, and medical cost measures. Each PCP will have targets for these metrics and will work toward them in collaboration with center and market leadership and care teams.

The PCP functions independently and collaboratively, works closely with managers, medical directors, risk and quality programs, and clinical meetings, while ensuring compliance with guidelines and company goals.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES

  • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult population.
  • The PCP takes full accountability for patient care and outcomes, and seeks specialist consultation when needed, while remaining involved in detailed patient care.
  • Engage with the hospitalist whenever one of their patients is hospitalized, regardless of hospitalist affiliation.
  • Lead assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic needs.
  • Lead the care team comprising a care promoter (medical assistant), care facilitator, and care coordinator for office‑visiting patients.
  • For patients unable to visit the office (hospital, SNF, LTC, or homebound), engage with transitional care teams, case managers, acute and transitional physicians, and other resources available in the market.
  • Assume an active role in center management, cover for other providers as needed, and contribute to recruiting patients and additional providers for the company.
  • Collaborate with or oversee an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions while employed with the company.
  • Perform other duties as assigned and modified at the manager’s discretion.

  • Evidence‑based medicine: Stay updated on clinical, nutritional, and lifestyle‑based interventions to improve outcomes.
  • Physician leadership: Continuously develop and improve leadership skills for the benefit of patients, teams, the center, and the company.
  • Quality focus: Work with care teams toward high‑quality outcomes and quality measures, and pursue systematic practice improvement.
  • Influence: Compartmentally and compassionately influence patients, teams, and self to achieve the best outcomes.
  • Self‑care: Ensure personal well‑being physically, mentally, socially, and spiritually to care for patients sustainably.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software.
  • Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training.
  • Fluency in English, verbal and written; additional language fluency may be required in some centers.
  • Use independent judgment in all aspects of clinical work.

PAY RANGE

$221,141 - $315,915 Salary

The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on factors such as experience, education, geographic location, and other relevant considerations. This position may also be eligible for bonuses or commissions.
