Under the direct authority of the Direction du pôle hébergement, you work with the professionals of the medical–social platform: the coordinating doctor, the social worker, health referents of establishments, the persons supported and their families or legal representatives, and in multidisciplinary collaboration with our establishments and external care partners (hospitals, CMPP).

Missions

• Inform, orient, and support the professionals of the establishments, notably the health referents, in linkage with the platform's doctor.
• Analyze the needs of the person taking into account its global environment and the actions already put in place (nurse, physiotherapist, speech therapist).
• Establish links with the various interveners with the person's agreement.
• Provide the most appropriate and adapted response.
• Lead towards common territory resources.
• Organize consultation meetings with all concerned parties.
• Facilitate the implementation of screening and prevention campaigns for establishment persons.
• Set up training or information actions for the professionals of the establishments.
• Formalize and maintain partnerships in connection with support to the health of persons.

Vous pouvez être amené à :

• Search for social aid in collaboration with the social worker.
• Help if needed to elaborate on the Personalized Care Project for the care part.
• Assist teams in filling out the computerised unique user file.

Profil

Candidate must be a state-qualified IDE, with a passion for caring for vulnerable persons and a desire to work in a team. The applicant should have pedagogical skills, be autonomous, and able to report on activities.

Compétences

• Infirmier‑diplôme d'État, débutant accepté.
• Possible travel to the communes of Colombes, Asnières-sur-Seine, Clichy and Gennevilliers (92) mainly.

Type de contrat

CDI, part‑time 24 h/semaine in daytime hours.

Rémunération

• Gross salary: monthly €1850.0 – €2999.0 over 12 months.

Expérience

• Beginner accepted.
• Bac+3, Bac+4, or equivalent Infirmier diploma IDE required (essential).
