In-Home Geriatric NP — Acute & Transitional Care

CareMore Health is seeking an Adult/Geriatric Nurse Practitioner for our growing home‑based care team serving the San Fernando Valley and Antelope Valley area. This full‑time, field‑based role focuses on delivering high‑touch care at patients' homes, including those recently discharged from hospital or skilled facilities.

You will manage an assigned panel of patients requiring acute and transitional care, order and interpret tests, develop care plans, and collaborate with physicians and care
