Hospice Field Staff RN/Case Manager
Hospice of Southern Illinois is a not‑for‑profit, community‑based hospice located in Belleville, IL. We provide compassionate care to terminally ill patients and their families in home or home‑like settings. The Hospice Field Staff RN/Case Manager reports to the staff RN and oversees patient care on the field and within the interdisciplinary team.
Position Summary
- Pay: $39.21 hourly. Full‑time, non‑exempt, 40 hours per week.
- Schedule: Monday‑Friday 8:30‑5:00 PM with rotational on‑call covering evenings, weekends, and holidays.
- Location: Belleville Patient Services Office; primary visits within St. Clair, Monroe, Randolph, and Washington counties.
- Valid driver’s license, automobile insurance, and reliable transportation required. Mileage reimbursement: $0.70 per mile.

Position Requirements
Education
- Graduate of state‑approved school of nursing.
- Associate Degree in Nursing or Diploma in Nursing required.
Experience
- One year experience in hospice preferred.
Other Qualifications
- Current Illinois nursing license.
- Certification in hospice and palliative care preferred.
- CPR certification preferred.
- Excellent leadership, managerial ability, clinical assessment, organizational and interpersonal skills.
- Knowledge of Medicare/Medicaid, hospice regulations, and third‑party payor expectations.
- Reliable transportation.
Physical Requirements
Repeats tasks of bathing, transferring, lifting, and turning patients. Requires bending at knee, squat‑bend, strength up to 100 lbs occasionally, 50 lbs frequently, and 20 lbs constantly. Clear vision >20 inches to <20 feet; hearing alarms, telephone, and tape recorder.
Working Conditions
Approximately 20% of time in an air‑conditioned environment, 80% in automobile or patient homes with variable heat/air conditioning. Exposure to odors, infectious fluids, pet hair, allergens, cleansing agent fumes, and communicable diseases.
Essential Functions of the Field Staff RN/Case Manager
- Maintains caseload; oversees patient and caregiver/family care.
- Provides direct patient care and participates in on‑call after‑hours care.
- Demonstrates RN clinical skills; conducts annual skills day demonstration.
- Completes physical assessment, medication review, and symptom/pain evaluation on each visit; enters medication orders timely.
- Communicates findings to physician and obtains care orders; orders pharmacy medications promptly.
- Conveys information to all disciplines via communicator, telephone, encrypted text messaging, and IDT meetings.
- Updates Plan of Care (POC) on each visit.
- Initiates, supervises, and updates Hospice Aide (HA) plan of care; supervises visits at least every 14 days per Medicare regulation.
- Knowledgeable in disease progression patterns, pain and symptom management, and engages patients/families in goal setting.
- Identifies physical, psychosocial, emotional, and spiritual needs; collaborates with IDT.
- Documents assessments systematically and timely per Policies and Standards of Performance.
- Oversees Licensed Practical Nurse (LPN) activity.
- Participates in on‑call after‑hour care; triages calls and applies critical thinking.
- In emergencies, demonstrates quick decision making and plan execution.
- Provides emotional, spiritual, psychosocial support; establishes therapeutic relationship.
- Shares updates at IDT meetings every 15 days, ensuring plan aligns with patient wishes.
- Works cooperatively within team; embraces change, manages stress, maintains positive attitude.
- Builds internal and external relationships; identifies referral issues; proposes solutions; participates in QAPI and performance improvement.
- Organizes work efficiently with minimal supervision; self‑starter, flexible, motivated.
- Contributes to new ideas and process improvements.
- Integrates Hospice core values into daily practice.
- Manages Paid Time Off appropriately; submits online requests timely.
- Maintains accurate timesheets per policy.
- Performs other duties as assigned.
